Medical Vein Clinic is a circulation disease treatment center in San Antonio, Texas, founded by board-certified radiologist Dr. John Hogg. We treat patients at two San Antonio locations: Stone Oak and Northwest San Antonio. Since opening in 2017, we have treated thousands of patients using minimally invasive, insurance-covered procedures, including Varithena and radiofrequency ablation. Founder, Board-Certified Radiologist and Vein Disease Specialist
John Hogg, MD,
DABR, DABVLM, RPVI, RPhS, RVT, RVS
Dr. Hogg is board-certified in diagnostic radiology (DABR) and vascular and lymphatic medicine (DABVLM), and holds registered certifications in vascular interpretation, phlebology (vein disease specialty), and vascular technology and sonography.
Dr. Hogg was inspired to open Medical Vein Clinic when he saw so many people with leg pain, swelling, and vein disease that too often went undiagnosed. Visiting a major hospital one day, the hospital’s administrator asked his opinion on what could be preventing a skin ulcer on her leg from healing. It was the size of an avocado and previous treatments had failed. The administrator had already consulted a cardiologist, an internist, and every department head at the hospital, yet they were unable to pinpoint the cause. Dr. Hogg immediately recognized the wound was caused by the lack of oxygen to her legs due to varicose veins. When the head of a hospital couldn’t get a proper diagnosis and treatment, he realized that ‘his calling’ was in opening Medical Vein Clinic to help thousands of others suffering needlessly.
MVC’s founder took a winding road to becoming a doctor. His first undergraduate degree was in accounting and finance from Baylor University. After graduating and passing the CPA exam, he worked for an accounting firm in Dallas but noticed his friends in medical school had much more interesting jobs. He started volunteering in a surgical ICU unit at Parkland Hospital and loved it so much that, with great determination and focus, he returned to college at SMU for his pre-med courses. Dr. Hogg went on to receive his medical degree from Baylor College of Medicine in Houston and did his residency in diagnostic radiology at Emory University in Atlanta.
This small-town Texas native made a name for himself in Gilmer (two hours east of Dallas), as the tall, skinny guy who ate more sweet potato pie, faster, than others twice his size at the annual Yamboree’s contest. Dr. Hogg has loved helping people since he was a young man, attaining the highest rank in Boy Scouts, Eagle Scout, and volunteering throughout college and beyond.
When he’s not in the clinic, you can find Dr. Hogg with his hands in the dirt in his garden, nurturing palm trees and orchids which he brings to the clinic to share with patients and staff. You might spot him driving around town with his three Vizsla's (red Hungarian Hunting dogs), joyfully hanging out of his truck’s windows.
Board-Certified General Surgeon with 20+ Years in Minimally Invasive Care
David Gutierrez, MD FACS Physician
Dr. Gutierrez is a Fellow of the American College of Surgeons (FACS), the highest recognition in surgical practice, with board certification in general surgery and a specialty focus on minimally invasive and aesthetic procedures. Dr. Gutierrez brings more than two decades of experience in minimally invasive and aesthetic procedures.
When this Desert Storm veteran, instrument-rated pilot, amateur race car driver, and highly-skilled general surgeon landed at Medical Vein Clinic, he found a clinic home that offered the practice and purpose he wanted in his life. “Everybody here loves coming to work to make our patients’ lives better,” Dr. Gutierrez happily observed. He received his undergraduate degree in biology from the University of Miami, then went on to medical school at Rutgers New Jersey Medical School in Newark, New Jersey. Dr. Gutierrez completed his surgical residency at the University of South Florida in Tampa, where he achieved the position of Chief Resident in Surgery in his last year there.
Before pursuing a career in medicine and surgery, Dr. Gutierrez served four years in the US Navy as an avionics technician aboard an aircraft carrier, working with the nation’s most sophisticated jets. He had wanted to be a Navy pilot but lacked the perfect vision the service
requires, so turned his sites to flying private aircraft and, then, medicine. For years, he married the two passions, volunteering ferrying patients for Pilots for Patients, taking them from rural Louisiana to MD Anderson Cancer Center in Houston.
The son of Cuban exiles and fluent in Spanish, Dr. Gutierrez was born in New York City and raised in Miami. He’s been married for 35 plus years to a woman he calls his “guiding light.” They met as kids in Miami as teenagers at the Burger King where they both worked. He has two grown daughters and a son who is training to be a pilot, much to David’s delight.
“I’m happiest when I’m caring for patients and giving them what they need. And, they are grateful for the improvements in their health when they get their veins repaired. Their gratitude feeds me like a paycheck.”
Board-Certified Vascular Surgeon, Fellowship Trained in Vascular and Endovascular Surgery
Thomas J. Gianis, II, MD, FACS
FSVS, DABVLM, RPVI, RVT
Dr. Gianis is a board-certified vascular surgeon and Fellow of both the American College of Surgeons and the Society for Vascular Surgery, with additional board certification in vascular and lymphatic medicine and registered credentials in vascular interpretation and technology.
He received his undergraduate degree from Vanderbilt University and his medical degree from Ross University School of Medicine. Dr. Gianis’ surgical training includes a general surgery internship at Vanderbilt University, a surgical residency at Memorial University Medical Center in Savannah, Georgia, and a fellowship in vascular and endovascular surgery at the University of Virginia Medical School - bringing well over a decade of surgical training and practice to his role at MVC.
At the clinic, he thrives on evaluating and treating patients, and improving their vein and leg health. This New Jersey native is happy to make San Antonio home, noting the people, weather, and Southern hospitality. When he’s not at work, Dr. Gianis enjoys spending time with his wife and four young children, golfing, fishing, reading, and skiing. He’s also a die-hard University of Alabama and NY Jets football fan who doesn’t miss a game.
Physical Therapist, Registered Vascular Technologist, and Certified Lymphedema Specialist
Joshua Trock, DPT, RVT, CLT-LANA, CEWS
Dr. Trock is a Doctor of Physical Therapy and Registered Vascular Technologist who holds national certifications in lymphedema therapy (CLT-LANA) and edema and wound care (CEWS), uniquely combining vascular diagnostics with advanced rehabilitative and lymphatic care. Dr. Trock has over a decade of experience and is a sought-after national speaker on lymphatic disease.
He brings a unique, multidisciplinary approach to the Medical Vein Clinic, bridging the gap between vascular diagnostics and advanced rehabilitative care. As a Doctor of Physical Therapy and a Registered Vascular Technologist (RVT), Dr. Trock is one of the few clinicians in the region capable of integrating diagnostic ultrasound with specialized theraputic interventions.
While edema (swelling) from venous insufficiency is a primary driver of vascular disease, Dr. Trock’s expertise extends far beyond standard vein care. He is nationally recognized subject matter expert in Lymphedema, Lipedema, and chronic wound management. His clinical focus includes:
- Lipedema & Lymphedema Mastery: Providing LANA-certified, specialized counseling and decongestive therapy for complex lymphatic disorders.
- Wound Care Specialty: Utilizing advanced clinical knowledge to accelerate healing in patients with venous and arterial ulcers and skin changes.
- Post-Surgical Edema Recovery: Designing custom protocols to reduce swelling and improve mobility following orthopedic or vascular surgeries.

A Functional Medicine Approach to Vascular Health Dr. Trock utilizes the principles of Functional Medicine to treat the whole person rather than just the symptoms. He emphasizes the critical link between physical function and vascular flow; by improving a patient's movement mechanics and musculoskeletal health, he optimizes the body’s natural ability to pump fluid and maintain healthy circulation.
Dr. Trock earned both his Bachelor’s and Doctorate degrees in Physical Therapy from UT Health San Antonio. He is dedicated to empowering patients through education, developing comprehensive home treatment plans that resolve pain, restore function, and improve quality of life.
Doctor of Nursing Practice Specializing in Vascular Disease
Jonathan Sidle, DNP, APRN, FNP-C
Jonathan Sidle is a Doctor of Nursing Practice and board-certified Family Nurse Practitioner (FNP-C) with advanced practice registered nurse credentials, specializing in the evaluation and treatment of vascular disease and chronic conditions. DNP Sidle brings more than a decade of treating patients for vascular disease and other chronic conditions.
Successful vein disease management can positively impact many aspects of a patient’s health, and this mission aligns with DNP Sidle’s passion for preventative medicine. At the clinic, he evaluates patients, develops treatment plans, and follows their progress back to health.
“In my previous years as a RN in hospitals and then as a DNP in primary care, I saw a lot of people with leg pain, ulcers, and swelling. But often they, as well as their health care practitioners, didn’t seem to understand what was going on. What drew me to MVC is the opportunity to improve someone’s quality of life. We have these sophisticated ultrasound machines and the expertise to diagnose and provide solutions.”
DNP Jonathan earned both his Doctor of Nursing and RN-BSN degrees from UIW. Originally from Ohio, he and his wife enjoy raising their three kids in San Antonio and attending church. Fun fact: He’s also an expert coffee roaster and has favorite methods for brewing a great cup. This java connoisseur’s go-to bean? Ethiopian, straight up.
Board-Certified Physician Assistant
Adams-Jeffrey Blanco, PA-C
Adams-Jeffrey Blanco is a nationally board-certified Physician Assistant (PA-C), trained to evaluate, diagnose, and treat patients across a range of vascular and vein conditions under physician collaboration.
After spending time practicing in primary care, San Antonio native Adams-Jeffrey Blanco finds it particularly rewarding to work at Medical Vein Clinic. In primary care he saw a lot of chronic disease admissions for high blood pressure and cholesterol, which contribute to artery and vein disease. “These patients’ conditions were routinely managed with medication which did not address the root cause. At Medical Vein Clinic, as soon as patients get vein treatments, they see a resolution of symptoms and their overall health improves. It’s very gratifying. In addition, this clinic (Stone Oak location) is one of the most beautiful places that I’ve ever seen.”
PA Blanco received his bachelor’s degree in clinical laboratory sciences from the UT Health Science Center, and a Master of Science in Physician Assistant studies (cum laude with honors) from the University of Texas at Rio Grande Valley, followed by his board certification as a Physician Assistant.
If there is someone who lives and breathes the health advice he dispenses, it’s PA Blanco. He has attained a purple belt and competes in Brazilian Jiu-Jitsu, something he was introduced to by a family member on a trip to Sydney, Australia. He and his wife joined a Jiu-Jitsu group in San Antonio and fell in love with it and practice together weekly for six hours or so. “It’s a challenging, humbling experience---kind of like chess for your body.”
Adams-Jeffrey also enjoys exercising with kettlebells and he is passionate about nutrition, fitness, hydration and sleep to optimize physical and mental health--something he talks to patients, friends and family about often. Reading books to improve his life in some way is another passion. His favorite titles are Atomic Habits and the long-time classic, How to Win Friends and Influence People by Dale Carnegie. And, more recently, when their two young children came along, parenting books got added to that reading list.
Bilingual Nurse Practitioner, Vein and Lymphedema Care
Isabel Hernandez, APRN, AGACNP-BC
Isabel Hernandez is an Advanced Practice Registered Nurse with board certification as an Adult-Gerontology Acute Care Nurse Practitioner (AGACNP-BC), specializing in complex vein, lymphedema, and lipedema care with a particular focus on older adult patients. NP Hernandez earned her bachelor's degree in nursing from UT Health Science Center, her clinical nurse specialist degree from the University of the Incarnate Word, and her nurse practitioner degree in 2024 from UT Health Science Center.
When she was 13, Isabel Hernandez’s neighbor in her hometown of Guanajuato, Mexico volunteered for the Red Cross. It was then Isabel knew she wanted to be a nurse, but as the oldest of eight siblings, with an infirmed mother, her father couldn’t afford to send her to medical school. Many years and kids later, this San Antonio transplant attended college to learn English and wrote an essay about her dream of a nursing career. One of her professors encouraged her to make her dream a reality. So, NP Hernandez studied at the UT Health Science Center and earned her bachelor’s degree in nursing, then obtained her clinical nurse specialist degree at the University of the Incarnate Word. In 2024, she proudly received her nurse practitioner degree from the UT Health Science Center.
This bilingual mother of four is a former marathoner who loves being outside in nature during her long runs and walks. Her nursing specialty is in gerontology care, and she especially enjoys calling up and checking on older people who have no one to talk to.
Working at Medical Vein Clinic has a very personal meaning for her. When she was a young girl in the small mountain town of Guanajuato, MX, her mom developed pneumonia which led to deep vein thrombosis (a blood clot) and years of suffering from lymphedema and lipedema. Medical professionals were stumped and could never diagnose or treat her mother. Her vein disease progressed into leg ulcers and frequent infections which were treated with so many antibiotics her kidneys failed. “When I talk to my patients, I sometimes get very excited explaining to them the importance of taking care of vein problems before they progress. My mom never got the treatment she needed, but now I can make sure others do. I’m so lucky to have this opportunity to help them.”

Mary Jane Avila, MSN, APRN, FNP-C
Mary Jane Avila is a Master's-prepared Advanced Practice Registered Nurse and board-certified Family Nurse Practitioner (FNP-C), evaluating, diagnosing, and monitoring vein disease patients across all stages of care. NP Avila brings more than two decades of clinical experience to her role at Medical Vein Clinic.
Avila was drawn to Medical Vein Clinic for their passion and dedication to educating and treating clients. “When patients understand their diagnoses, it increases their compliance and they have better health results,” she shared.
Growing up, Mary Jane wanted to be a physician but pursuing that was very expensive in her native Philippines. So, she altered her goals. She received a nursing degree and a Master of Health Professions in Education from the University of Santo Tomas in the Philippines. And, in 2024, she became an accredited Family Nurse Practitioner, earning her Master of Science in Nursing from Walden University in Minneapolis, MN.
Mary Jane is happily married and has two girls who, she will proudly tell you, are in the gifted and talented programs at their school. When she is not in the clinic, she gets joy from simple hikes in the mountains and as the go-to party decorator for friends and families. “I love it when guests are stunned. I like being of service in that way.”
