Pelvic Venous Disorder (PVD) Treatment in San Antonio — Minimally Invasive Relief for Chronic Pelvic Pain

Pelvic Venous Disorder (PVD) is a leading cause of chronic pelvic pain in women, affecting nearly one in three women during their lifetime. It occurs when ovarian and pelvic veins become dilated and form painful varicose veins inside the pelvis. Symptoms include persistent pelvic pain, pain during or after intercourse, bulging vulvar or thigh veins, bladder symptoms, and pain that worsens throughout the day. Medical Vein Clinic treats Pelvic Venous Disorder in San Antonio using a minimally invasive catheter procedure to stent or embolize the problem vein. The procedure takes about one hour, requires no hospital stay, and most patients resume normal activity the next day. Up to 85% of women find relief within two weeks of treatment. PVD is treated by Thomas Gianis, II, MD, FACS, FSVS, DABVLM, RPVI, RVT, CWSP, a board-certified vascular surgeon at Medical Vein Clinic. Most insurance plans cover treatment.

What is Pelvic Venous Disorder and What Causes It?

Pelvic Venous Disorder happens when ovarian and pelvic veins get dilated (congested) and form painful pelvic varicose veins. This condition can cause chronic pelvic pain in nearly a third of women during their lifetime, most commonly occurring between 20 and 50 years of age. After treatments, up to 85% of women find relief from symptoms within two weeks.

Common Symptoms of Pelvic Venous Disorder:

  • Worse pain when walking or standing, or as day progresses

  • Dull, aching pain or heaviness, occasionally sharp pains

  • Pain worsening with menstrual cycles

  • Pain during or after sexual intercourse

  • Bulging vulvar, vaginal, or upper thigh veins

  • Bladder and urinary tract symptoms

  • Irritable bowel (recurrent abdominal pain and diarrhea with intermittent constipation)

  • POTS (Postural Orthostatic Tachycardia Syndrome)

  • Headaches

  • Non-typical lower back pain

What to Expect During the PVD Procedure

During the procedure, Dr. Gianis guides a small catheter through a vein in your neck or groin to reach the affected pelvic veins. Using real-time X-ray imaging, he precisely delivers a stent to open a compressed vein or embolization coils to block a leaking vein, stopping the abnormal blood flow causing your pain. The approach used depends on your specific diagnosis and imaging results.

The procedure normally takes about one hour.  We use IV conscious sedation (you’ll be relaxed, but awake) during the procedure.  Because of the type of anesthesia used, you will need someone to drive you home and stay with you that day. Most patients go home within a few hours. You can resume light activity the following day and return to normal activity within one to two weeks.
