Certified Vascular Surgeon
Thomas J. Gianis, II, MD, RPVI
Dr. Gianis is a Board-Certified Vascular Surgeon at Medical Vein Clinic who brings great experience, professional recognition, and research to this role. He received his undergraduate degree from Vanderbilt University and his medical degree from Ross University School of Medicine. Dr. Gianis’s surgical training includes a general surgery internship at Vanderbilt University, a surgical residency at Memorial University Medical Center in Savannah, Georgia, and a fellowship in vascular and endovascular surgery at the University of Virginia Medical School.
At Medical Vein Clinic, he thrives on evaluating, treating patients, and improving their vein and leg health. This New Jersey native is happy to make San Antonio home now, noting the people, weather, and Southern hospitality. When he’s not at work, he enjoys spending time with his wife and four young kids, golfing, fishing, skiing, and reading historical fiction. He’s also a die-hard University of Alabama and NY Jets football fan who doesn’t miss a game. Dr. Gianis’s golden retriever, Cooper, is a testament to his fandom level, named after Alabama wide receiver and now NFL standout, Amari Cooper.
Doctor of Physical Therapy, Lymphedema Specialist and
Registered Vascular Technologist
Joshua Trock, DPT, RVT, CLT-LANA
Dr. Trock complements the clinic’s varicose vein treatment experts, extending their services to care for patients suffering from lymphatic disease. Edema (swelling) caused by varicose veins or venous insufficiency is the leading cause of lymphedema. In addition to being LANA certified in Lymphedema Therapy as a Doctor of Physical Therapy, Dr. Trock is also a Registered Vascular Technologist (RVT). He is dedicated to evaluating and educating patients along with developing home treatment plans to resolve vein pain and swelling associated with vein disease. He also provides specialty counseling and treatment for those suffering from lymphedema. Dr. Trock received his bachelors and doctorate degrees in physical therapy from UT Health San Antonio and has applied his lymphatic disease knowledge to patient care for more than a decade. He is a subject matter expert and speaks nationally on the topics of lymphatic disease and lymphedema.
Nurse Practitioner
Jonathan Sidle, DNP, APRN, FNP-C
DNP Sidle is a member of MVC’s leadership team, brings more than a decade of treating patients for vascular disease and other chronic conditions. Successful vein disease management can positively impact many aspects of a patient’s health and this mission aligns with DNP Sidle’s passion for preventative medicine. At the clinic, DNP Jonathan evaluates patients, develops treatment plans and follows their progress back to health.
“In my previous years as a RN in hospitals and then as a DNP in primary care, I saw a lot of people with leg pain, ulcers and swelling. But often they, as well as their health care practitioners, didn’t seem to understand what was going on. What drew me to MVC is the opportunity to improve someone’s quality of life. We have these sophisticated ultrasound machines and the expertise to diagnose and provide solutions.”
DNP Jonathan earned both his Doctor of Nursing and RN-BSN degrees from UIW. Originally from Ohio, he and his wife enjoy raising their three kids in San Antonio and attending church. Fun fact: He’s also an expert coffee roaster and has favorite methods for brewing a great cup. This java connoisseur’s go-to bean? Ethiopian, straight up.
Nurse Practitioner
Adam Sahyouni, MSN, APRN, AGACNP-BC
NP Sahyouni joined the Medical Vein Clinic leadership team in 2024 as a Nurse Practitioner. He puts his eight years of nursing expertise to help MVC patients. He enjoys listening to their challenges, coming up with plans to eliminate those problems, and tracking their progress. “I got into medicine because I love interacting with patients and being of service.”
He received his BSN in nursing from the Chamberlain College of Nursing and his MSN with a specialty in adult gerontology and acute care from the UT Health Science Center. In 2020, Sahyouni was named a Healthcare HERO by the Texas Organization for Nursing Leadership.
A Texas native, he and his wife enjoy raising their blended family of five kids (three daughters and two sons). When Adam is not in the clinic, he enjoys watching football, playing and watching golf, and learning. “I am a lifelong student and always want to be expanding my knowledge to help our patients.”
Functional and Lymphedema Specialist
Tabitha Castillo, PT, DPT, CLT
As a Certified Lymphedema Therapist (CLT) as well as a Doctor of Physical Therapy (DPT), Castillo joined Medical Vein Clinic to focus on treating patients with lymphedema. Working closely with those experiencing swelling in their legs, she assesses their health, and shares treatments and practices to improve their mobility and quality of life. Dr. Castillo earned her doctorate in physical therapy degree from The University of Texas Health Science Center. She has a decade of rehabilitation experience, treating a variety of medical conditions in hospital and outpatient clinics.
“The joy and happiness I see on patients’ faces when their leg issues are diagnosed and resolved is so rewarding,” Castillo shared. When she’s not at the clinic helping patients, Castillo likes to read sci-fi books, work out at the gym, watch movies, and spend time with her cat, Matcha.
